Depleted ovary symptoms

The symptomatology of this disease most often begins to develop at the age of 35-38 years.

The main symptom is the absence of menstruation. This fact is preceded by a change in the amount of menstrual flow (their decrease) and the irregularity of the menstrual cycle.

Other symptoms include:

  • Hot flushes to the head and torso, severe sweating, various irritations of the skin on the chest and neck and their redness;
  • Dryness and discomfort in the genitals;
  • Decreased attraction to a partner;
  • Unreasonable irritability, mood changes very often, depressive states are possible;
  • Decreased performance, fatigue, memory impairment.

Diagnostics of the ovarian failure syndrome

The diagnosis of this syndrome can be made on the basis of:

  • The patient's complaints, such as: cessation of menstruation, hot flashes, depression and others;
  • Diseases with which the patient had been ill earlier, operations and injuries, heredity;
  • Analysis of the menstrual cycle;
  • Analysis of diseases of the gynecological group suffered by a woman, operations on the organs of reproduction, the course of pregnancies and childbirth in the previous time;
  • Ultrasound results: the state of the uterus and determination of the reduction in the size of the ovaries, the presence or absence of follicles, a decrease (if any) in the thickness of the endometrium;
  • Laparoscopic data: the state and size of the ovaries, the presence of follicles in them;
  • The results of the analysis for sex hormones: a decrease in the level of progesterone and estrogen and an increase in the pituitary hormone.

Treatment

Attempts to conceive in patients who have been diagnosed with this diagnosis are rarely successful. However, in some isolated cases, hormone therapy is used, which may help restore normal ovarian function and induce ovulation. The severe depletion of the follicular apparatus that occurs with this syndrome leads to the conclusion that stimulation of the depleted ovaries is inappropriate. Then how to restore ovarian function and is it possible to get pregnant after normalizing ovarian function?

To restore ovarian function, hormone replacement therapy is used, up to the onset of natural menopause. Premature ovarian failure (PJF) was first described in 1942, after which a variety of terms were assigned to this pathology (premature menopause, malnourished ovarian syndrome, resistant ovary syndrome, hypergonadotropic amenorrhea, autoimmune oophoritis, and others), many of them are still used today.

Currently exactly the termpremature ovarian failure (premature ovarian insufficiency)is used by the leading professional societies of the USA, EU countries and other countries, and it is he who is recommended for use in clinical practice.

In its 2016 clinical guidelines, the European Society for Reproduction and Embryology (ESHRE) defines premature ovarian failure (PIAN) - clinical syndrome, the main manifestation of which is the cessation of ovarian function before the age of 40, characterized by menstrual dysfunction (rare scanty menstruation or their complete cessation), increased pituitary hormones, which stimulate the ovary and a decrease in the female sex hormone estradiol.

With chromosomal reasons, it happens that menstruation does not start at all. But most often PYAN is manifested by secondary amenorrhea (menstruation was, and then stopped suddenly or gradually).

Secondary amenorrhea can occur suddenly or after a period of rare menstruation; there are also common options for the absence of menstruation recovery after taking hormonal contraceptives or childbirth (neither contraceptives nor childbirth are to blame!).

Another important point about the PYAN clinic, which not even all doctors know: PYAN is not menopause and in most cases young women, even with very low levels of sex hormones, do not have symptoms like women in menopause (hot flashes, night sweats, etc.) etc.).

But problems such as a decrease in bone density and problems with the heart and blood vessels are very relevant for young women with PYAN.

PIAN symptoms

  • Infrequent menstruation, a decrease in the likelihood of conceiving a pregnancy, amenorrhea - the absence of menstruation for 4-6 months.

  • Increased blood pressure
  • Hot flashes, night sweats (women with PYAN up to 40 years old are observed much less often than menopausal women!).

How do doctors diagnose PYAN?

First, they exclude other possible reasons for the absence of menstruation, then they examine the levels of female sex hormones (reduced) and pituitary hormones that stimulate the ovaries to work (increased), as well as markers of the ovarian reserve (Antimüllerian hormone, inhibin B, the number of small antral follicles by ultrasound - reduced ). By the way, the study of hormones (FSH and estradiol) is carried out twice with an interval of 4 weeks.

If the results are in favor of PYN, then they try to look for the reasons (genetic studies, antibodies, etc.), and also examine the state of the bone tissue and the cardiovascular system for the timely prevention of complications.

Is it possible to somehow determine who will have it?

The problem is that there are no reliable methods for forecasting PYAN yet. But they are actively looking for!

Here's what we can do now:

  • When consulting on family planning / contraception, we always educate women who are inclined to long-term contraception about the PIT problem.
  • We know some risk factors (early menopause in the mother, diseases in the family, indicating the possibility of a genetic cause of PNI, surgery or cancer treatment in a woman's life ....). On their basis, we can identify women who need to be especially alert about the possibility of early termination of ovarian function.
  • An expert-class ultrasound and hormonal testing can help the supervising gynecologist diagnose PUP in the early stages. This does not work while taking combined hormonal contraceptives.

How is PIAN treated?

Unfortunately, modern medicine does not have ways to cure this disease, even in most cases when its cause can be found (but it is still advisable to look for the reasons).

Despite the fact that the facts of spontaneous return of the ovaries to work and even pregnancy with PNI have been documented, women with this disease need constant medication to compensate for the lack of female sex hormones. This is necessary for the normal quality of life, health of the heart, blood vessels, bones, and the nervous system.

With this disease, it is impossible to replace hormonal preparations with herbal or vitamins. Although both are also used, but only an addition. In some cases, it is necessary to use drugs that act like male sex hormones.

A gynecologist-endocrinologist, with the involvement of, if necessary, specialists of related specialties, develops an individual plan for the observation of a woman with PYAN.

Women with PYAN need lifelong observation by a team of specialists, including a gynecologist, if necessary, a reproductologist, geneticist, endocrinologist, cardiologist, osteoporosis specialist, psychologist, social worker.

A healthy lifestyle is very important: smoking cessation, limiting alcohol, regular physical activity, good nutrition.

Managing women with PYAN includes solving the following tasks:

  • elimination of estrogen deficiency, regardless of the presence of symptoms of their deficiency,
  • solving reproductive problems,
  • monitoring and prevention of osteoporosis,
  • monitoring and prevention of cardiovascular diseases,
  • elimination and prevention of urinary symptoms, sexual dysfunction.

Pregnancy with PYAN?

This is perhaps the biggest challenge in modern reproductive medicine.

Various scientific groups are working to develop protocols for stimulating ovulation with very small numbers of immature follicles. There is also a lot of research going on about how the known gene causes of PNI can be influenced.

All this is encouraging, but so far the only reliable method of childbirth for women with PJA is the use of a donor oocyte.

In 5-10% of cases with PN, spontaneous restoration of fertility and even pregnancy is observed. But there is still no explanation for this phenomenon, as well as the possibility of predicting it.

There is other good news

  • Scientific research shows that it is possible to develop new eggs during a woman's life. It is known that the ovarian reserve (the number of immature follicles containing eggs) is laid in every woman even before birth and decreases throughout life, determining the age of menopause. But this long-standing paradigm was shaken by the results of research, begun back in 2012, which showed the possibility of egg production from stem cells in the ovaries of women after birth.
  • Modern medicine is increasingly mastering the method of three-dimensional printing: now, with the help of 3D printers, they have already learned how to make artificial auricles, heart valves, and bone-like implants.

In the most successful variants, the follicles continued to develop for 8 days, it was these semi-artificial ovaries that were transplanted into seven mice to replace their own. After transplantation, the blood vessels penetrated the gelatinous base, supplying the follicles with nutrients and hormones, so that eventually the eggs passed all the necessary stages of development and became ready for fertilization. Premature ovarian failure is a complex, severe disease that affects all aspects of a woman's life. But as evidenced by reports in the Internet communities of women with PNI, as evidenced by our experience of treating patients with this problem, a full life with it is entirely possible. In women, the ovaries are primarily affected. Their aging is manifested by a number of disorders - from the failure of the menstrual cycle to the appearance of cysts and fibroids. Chronic fatigue, brittle nails, or sagging skin are common indicators of ovarian disease.
